RET/PTC3 translocation in a rare hemorrhagic brain metastasis of papillary thyroid cancer post Chernobyl radiation affects vessels ultrastructure

Abstract Background Slow progression and good prognosis are the usual characteristics of papillary thyroid carcinoma (PTC). The presence brain metastases (0.4–1.2%) is suggestive a worse prognosis. RET/PTC rearrangements were particularly prevalent in PTCs developed after Chernobyl nuclear accident. Case description A 50-year-old woman born Slovakia, exposed to radiation resulting from accident at power plant, affected since 2017 by cancer therapy our hospital, experimented cerebral hemorrhagic metastasis. Biopsy analyses revealed RET/PTC3 rearrangement, so aim was find possible morphological relation between metastasis translocation. Results Immunohistochemical analysis showed diffuse intense positivity for VEGF endothelial cells neoplasm’ vascular network. Transmission electron microscopy images vessels with unorganized pattern uneven diameters. In particular, (MECs) irregular shape size, thickened cytoplasm swelling endoplasmic reticulum. MECs organized monolayers or multiple layers, surrounded but unstructured extracellular matrix. Absence strong junctional complexes among resulted further weakened wall. Conclusion translocation causes overexpression via STAT3 signaling cascade increased amount adds greater VEGFRs expressed MECs.
